British Cardiovascular Society (BCS) and British Society for Heart Failure (BSH) joint statement on ACEi or ARB in relation to COVID-19

'There is no evidence to support the assertion that treatment with ACEi or ARB could predispose individuals to adverse outcomes; and that both organisations share the view of the European Society of Hypertension and the Renal Association that patients should continue treatment with ACEi and ARB unless specifically advised to stop by their medical team'

DCHS Care Home COVID Support Service

A multidisciplinary team consisting of registered staff including Nursing, Physiotherapy, Occupational Therapy, Speech and Language Therapy, Pharmacy, Podiatry and Continence services will provide a virtual service for non-urgent needs to support the care home provider in the ongoing care of residents during the pandemic

DDCCG Homeless patient registration primary care access during COVID-19

Derby and Derbyshire CCG will facilitate a service to support clinical decision-making during the COVID-19 pandemic for the homeless population who have been placed in designated hotels in the Derby City and Derbyshire County area. The aim is to manage homeless patients remotely whilst they are living at designated hotels. This supports reducing direct patient contact and thereby infection risk, and unnecessary hospital admission.

Dexamethasone in the treatment of COVID-19

Dexamethasone has been demonstrated to have a clear place in the management of hospitalised patients with COVID-19. There were no excess harm identified in using this dose of dexamethasone in this patient population. Clinicians should therefore consider dexamethasone for the management of hospitalised patients with COVID-19 who require oxygen or ventilation. Out of hospital treatment is not appropriate.

The Department of Health and Social Care (DHSC) and MHRA have announced restrictions of dexamethasone on parallel exported from the UK. View the full export parallel ban list here.

Through our Patient Safety Collaboratives, the AHSN Network have been working to provide useful information and resources for care homes to support their response to COVID-19.
 
Resources to highlight are: 

A national review of medication in care homes and a blog from one its authors, Tony Jamieson, on the background to the research. 

Free online video training for care home staff – a series of 14 short videos describing how to take measurements from residents correctly, spot the signs of deterioration, and prevent the spread of infection.

MHRA April 2021 - Inhaled Budesonide for Adults (50 Years and Over) with COVID-19

A Covid-19 Therapeutic Alert CEM/CMO/2021/011 issued alongside an interim position statement.

Inhaled budesonide is not currently being recommended as standard of care but can be considered (off-label) on a case-by-case basis for symptomatic COVID-19 positive patients aged 65 and over, or aged 50 or over with co-morbidities, in line with the published Interim Position Statement.

NHS Education for Scotland - Management of Acute Dental Problems During COVID-19 Pandemic

Patients should be encouraged to manage their symptoms at home where possible as treatment options are severely restricted at this time. Mild and moderate dental symptoms should be managed remotely by providing advice and analgesics and/or antimicrobials where necessary.

NHS Volunteer responders information for health professionals

The volunteer initiative is being delivered by NHS England and NHS Improvement in partnership with Royal Voluntary Service. NHS Volunteer Responders have been mobilised to help support vulnerable individuals who are self isolating. Priority will be given to those identified as most risk from COVID-19 and asked to self-isolate at home for an extended period, and those who health practitioners and local authorities consider to be vulnerable.

NPA COVID-19: Guidance on dealing with patient-returned / unwanted medicines (England)

Community pharmacies routinely accept unwanted/out of date/waste medication for disposal from patients and households, as this is an NHS contractual requirement. This service has not been suspended during the current COVID-19 pandemic.

For patient safety reasons, it is important that patients and households remain able to return unwanted medicines to the pharmacy for disposal during the COVID-19 pandemic, to avoid potential risk of accidents, overdoses and misuse

This document sets out the interim clinical commissioning position for the use of remdesivir for patients with COVID-19.

RPS - Changing the intervals of instalments for controlled drugs during the pandemic

Legislation has been amended, informed by the advice of the Advisory Council on  the Misuse of Drugs, to enable pharmacists to vary the frequency of the supply of Schedule 2 and 3 CD instalments, without the need for a further prescription.

Supply of non-VKA oral anticoagulants (NOACs) during COVID-19

During COVID-19 an NHSE guideline advises that patients requiring initiation with an anticoagulant should be prescribed a NOAC, rather than warfarin, wherever clinically appropriate in order to reduce the requirement for INR testing - edoxaban remains our first choice for NVAF patients.
