GREY:6 Limiting angina (as confirmed by a cardiologist) with no options for revascularization and despite conventional medical treatment with at least a beta-blocker (or rate limiting calcium channel blocker if a beta-blocker not tolerated or contra-indicated) and a long acting nitrate (if tolerated). The drug should not be used in patients with chest pain which is atypical or non-anginal, nor in those who have not had demonstrable myocardial ischaemia'. One month initially and only continued if there is clear symptomatic benefit.
